[NAME] Medical Imaging Center is a participating provider in an extensive national network of insurance and managed care organizations. We are able to submit your bill for services as required by your insurance carrier. At registration, we may photocopy your insurance card and ask for any information pertinent to your insurance coverage. Some of our services may require a Pre-Authorization from your insurance carrier. This is generally done by the Physician referring you for the service. We will need to verify a pre-authorization (referral) is in place before we can provide those services. Radiology services generally requiring pre-authorization are: CT Scans MRI / MRA Nuclear Cardiology Bone Densitometry Tests A majority of insurance carriers require a co-payment for services to be made by the patient. Please call your insurance carrier at the number on the back of your insurance card to verify the co-pay and/or deductible amount you will be responsible for. Co-payments will be collected at the tine of service.
